Directions: Please fill out the information below. Once you click “Submit” at the bottom of the form, you will be taken to a confirmation page. That page will have two forms for you to print: Physical Examination form (1) and Medical Release form (2). Your application will not be complete until these two forms are signed and returned to the Director or Assistant Director.
